Duties Assist individuals with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts. Monitor patients' vital signs, observe for changes in health status, and report findings accurately while adhering to HIPAA regulations to protect privacy. Prepare nutritious meals and assist with meal consumption, ensuring dietary needs are met in accordance with care plans. Administer medications safely following prescribed instructions and document medication administration precisely. Support residents with memory care needs, including dementia and Alzheimer's care, utilizing specialized techniques to enhance safety and comfort. Maintain a clean and safe environment by performing routine cleaning tasks and ensuring infection control standards are upheld. Provide transportation assistance for appointments or community outings while ensuring resident safety during transfers and travel. Engage residents through meaningful communication, recreational activities, and socialization to promote mental well-being. Follow all care plans meticulously, including documentation of patient observations, progress notes, and incident reports to ensure continuity of care. Experience Prior experience working with individuals with developmental disabilities, intellectual disabilities support, or neurodevelopmental disorders is highly valued. Experience in group home settings, assisted living facilities, nursing homes, or home health environments is preferred. Knowledge of patient monitoring techniques such as vital signs measurement and patient observation is essential. Familiarity with medication administration procedures and HIPAA compliance is required to ensure confidentiality and safety. Certification in CPR and First Aid is necessary; ongoing training in dementia care, Alzheimer's care, hospice care, or geriatrics is a plus. Demonstrated ability to assist with ADLs using manual handling equipment like Hoyer lifts or patient transfer methods is important for success in this role.
