Responsibilities Assist residents with Activities of Daily Living (ADLs), including personal hygiene, grooming, dressing, and mobility. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare teams. Support medication administration following care plans while adhering to HIPAA regulations for patient privacy. Prepare nutritious meals and assist with meal consumption, ensuring dietary needs are met. Operate patient transfer methods such as Hoyer lifts and manual handling techniques safely to prevent injury. Maintain cleanliness of living spaces by performing routine cleaning tasks in accordance with health standards. Document resident progress, update care plans, and communicate effectively with clients and team members about individual needs. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders such as autism is a plus. Ability to perform patient observation, monitor vital signs, and recognize signs of distress or medical emergencies. Current CPR, First Aid, Certified Medication Technician (CMT), and Mandt certifications are required. Hospice care certification is preferred. Comfortable with manual handling techniques, including heavy lifting and patient transfers using equipment like Hoyer lifts. Strong communication skills for effective client interaction and teamwork within a supportive environment. Demonstrated understanding of residents' rights, HIPAA compliance, and confidentiality standards.
