Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature to ensure patient health status is maintained. Administer medications following medical documentation protocols and home healthcare regulations while maintaining strict patient confidentiality in accordance with HIPAA standards. Provide specialized care for clients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care and behavioral support. Support clients with medical procedures such as catheter care, wound dressing, and basic life support or CPR when necessary. Maintain detailed medical records using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR systems), ensuring all documentation is accurate and up-to-date. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment to promote overall well-being. Communicate effectively with clients and their families to address needs, preferences, and concerns while respecting cultural sensitivities. Adhere to all home health agency policies and home health care assistance standards to deliver consistent, high-quality care. Qualifications Proven experience working with individuals with disabilities or in assisted living environments; experience in geriatrics or long-term care preferred. Knowledge of medical terminology, medication administration protocols, and home health regulations. Certification in first aid and CPR; additional certifications such as Basic Life Support (BLS) are highly desirable. Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts or other mobility aids. Strong patient observation skills to detect changes in condition or behavior promptly. Excellent communication skills for effective client interaction and documentation accuracy. Experience working with EMR/EHR systems for medical documentation is preferred. Valid driver's license and reliable transportation may be required depending on client location.
