Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure their comfort and dignity. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) systems. Support medication management by administering prescribed medications following medical terminology guidelines and home healthcare regulations. Provide specialized care including Alzheimer's care, dementia care, hospice support, and end-of-life assistance with sensitivity and professionalism. Assist with mobility and patient transfer methods utilizing equipment like Hoyer lifts or safe transfer techniques to prevent injury. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients. Maintain strict confidentiality of patient information in compliance with HIPAA regulations while fostering open communication with clients and their families. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly desirable. Demonstrated expertise in patient observation, medical terminology, catheter care, and vital signs monitoring. Hands-on experience with EMR/EHR systems for accurate medical documentation and record keeping. Knowledge of home health care assistance including medication administration, patient transfer methods, and basic life support (BLS) or CPR certification. Experience working with individuals with developmental disabilities, geriatrics, or memory care needs such as Alzheimer's or dementia is preferred. Familiarity with home health agency operations, hospice care protocols, heavy lifting techniques, and home & community care standards. Strong communication skills to effectively engage with patients, families, and healthcare teams while maintaining patient confidentiality under HIPAA guidelines.
