Job Description

Job Overview We are seeking a compassionate and dedicated Personal Care Assistant to join our team in providing exceptional support and care to individuals in various settings, including home health, assisted living, nursing homes, and group homes. In this vital role, you will empower clients to maintain their independence, improve their quality of life, and achieve their personal health goals. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene Monitor patients' vital signs such as blood pressure, pulse, and temperature to ensure their well-being Support medication administration following care plans and HIPAA regulations for patient privacy and confidentiality Help with mobility and transfer techniques using equipment like Hoyer lifts or patient transfer methods to ensure safety during movement Prepare nutritious meals tailored to dietary needs and assist with meal consumption when necessary Conduct patient observation and monitor changes in condition or behavior, especially for individuals with dementia or neurodevelopmental disorders Maintain cleanliness by performing light housekeeping tasks such as cleaning patient areas and ensuring a safe environment Support patients during medical procedures or treatments under the supervision of nursing staff or healthcare professionals Assist with transportation for appointments or community outings using personal transportation assistance skills Document care activities accurately in compliance with care plans and healthcare regulations such as HIPAA Skills Proven experience working with individuals with disabilities, developmental disabilities, or neurodevelopmental disorders such as autism or intellectual disabilities support Strong knowledge of patient care procedures including first aid, CPR, and basic nursing skills Experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Ability to perform manual handling tasks safely, including heavy lifting and patient transfers using appropriate techniques like Hoyer lifts Excellent client communication skills with a compassionate approach toward residents' needs and preferences Familiarity with dementia care, Alzheimer's care, memory care strategies, and hospice care practices Knowledge of medication administration protocols and the importance of adhering to care plans Ability to observe patients carefully for changes in condition or behavior that may require escalation or intervention Competence in maintaining documentation while respecting patient privacy laws such as HIPAA Strong organizational skills for managing multiple responsibilities efficiently in fast-paced environments Join us if you're eager to provide meaningful support through caregiving roles that truly make a difference!
