Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and observe patients' health status by checking vital signs and reporting changes promptly Administer medications and manage medical documentation in compliance with HIPAA regulations and EMR/EHR systems Provide specialized care such as Alzheimer's, dementia, memory care, hospice, and catheter care tailored to individual needs Support clients with meal preparation, feeding, laundry, cleaning, and maintaining a safe home environment Document patient interactions accurately in medical records and communicate effectively with healthcare teams Use medical terminology confidently to record observations and assist with medical procedures as directed Experience Proven experience as a Home Health Aide or in a similar caregiving role within home care or long-term care settings Prior work in assisted living facilities or nursing homes is highly desirable Familiarity with working with individuals with developmental disabilities or disabilities in general Hands-on experience with medical equipment such as Hoyer lifts and blood pressure cuffs Knowledge of EMR/EHR systems for accurate documentation of patient care activities Demonstrated ability to perform patient observation, medication administration, and dementia or Alzheimer's care effectively Valid CPR certification and understanding of medical terminology are required for this role Join us to empower individuals through compassionate caregiving that enhances their quality of life.
