Job Description

Role:
Medical Social Worker (MSW) in Home Care, responsible for providing psychosocial assessment, counseling, crisis intervention, and referral services.
Key Duties:
Conduct initial psychosocial evaluations through patient interviews. Employ counseling techniques to help patients and families understand, accept, and follow care recommendations. Intervene effectively in crisis situations using crisis intervention strategies. Identify and document psychosocial, clinical, and financial barriers to healthcare transitions. Collaborate with management and healthcare teams to optimize resource utilization for positive patient outcomes. Stay informed about community resources for social, emotional, and financial assistance. Report suspected cases of abuse, neglect, or exploitation. Participate in audits and reviews to ensure compliance with laws and regulations.
Qualifications & Skills:
Master's Degree in Social Work. At least 1 year of social work or relevant mental health experience. Effective communication skills and ability to work with diverse populations. Knowledge of patient rights, laws such as HIPAA, and psychological principles. High emotional intelligence and empathy. Ability to logically analyze and solve problems.
Preferred:
Licensed Social Worker (LSW) in Pennsylvania.
Additional Information:
Must be able to lift 7 lbs., sit for extended periods, and perform repetitive motions. The role emphasizes teamwork, compliance, and compassionate patient care.
