We are currently seeking a dedicated Physical Therapist to join our growing team. Why Join Nightingale? Competitive per-visit compensation Flexible scheduling Mileage reimbursement Up to 3 weeks Paid Days Off (PDO) per year One-on-one patient care Referral bonuses up to $500 Annual performance evaluations with merit increases Comprehensive full-time benefits including Medical, Dental, Vision, Life Insurance, and 401(k) match after one year Careport Program - remotely monitor patients' home exercise programs to enhance outcomes Your Role Evaluate patients and initiate therapy within 72 hours of referral Develop and implement individualized Plans of Care Supervise Physical Therapist Assistants (PTAs) and/or Home Health Aides (HHAs) Coordinate care with physicians, caregivers, and the interdisciplinary rehab team Maintain accurate documentation and timely visit notes Conduct supervisory visits and manage discharge planning Provide in-service education and participate in team meetings Ensure HIPAA compliance and adherence to agency policies Engage in ongoing professional development and collaboration Qualifications Current Physical Therapist license in Connecticut Minimum of 1 year PT experience; Home Health and OASIS experience preferred Experience with WellSky (Kinnser) software preferred Physical exam within the last 12 months Valid TB test (within 12 months) and current CPR certification Valid driver's license Ability to attend a 1-2 day orientation in Stamford, CT Availability Monday through Friday, 8:00 AM - 5:00 PM Make a Difference — One Patient at a Time Join a team that values your expertise, supports your professional growth, and recognizes the meaningful impact you make every day.
