Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide high-quality support and assistance to individuals in various care settings. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper manual handling and patient transfer methods such as Hoyer lifts when necessary. Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately while respecting resident's rights and maintaining HIPAA compliance. Support medication reminder. Ensuring proper documentation and adherence to care plans. Prepare nutritious meals aligned with dietary restrictions and preferences, and assist residents with meal consumption to promote optimal health. Maintain a clean and safe environment through regular cleaning duties, infection control practices, and adherence to safety protocols including first aid and CPR readiness. Document patient observations, update care plans regularly, and communicate effectively with team members regarding resident progress or concerns. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
