Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques. Monitor patient health by observing vital signs, noting changes in condition, and reporting findings according to care plans and HIPAA regulations to ensure continuous quality care. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition. Support residents with medication administration under supervision, ensuring adherence to prescribed schedules and safety protocols. Maintain a clean, safe environment by performing light housekeeping duties such as cleaning resident areas, laundry, and sanitation tasks aligned with infection control standards. Document care activities accurately in resident records, update care plans regularly, and communicate effectively with team members about each resident's needs. Provide compassionate emotional support and engage residents in social activities to foster mental well-being and reduce feelings of isolation. Skills Proven experience working with individuals with disabilities, including developmental disabilities, neurodevelopmental disorders, or aging populations such as geriatrics or hospice care. Strong patient observation skills to detect changes in condition or behavior promptly. Knowledge of specialized care such as Alzheimer's care, dementia care, memory care, and resident's rights. Ability to perform heavy lifting safely using proper manual handling techniques; experience working with Hoyer lifts is a plus. Competence in patient monitoring, vital signs measurement, first aid, CPR certification, and emergency response procedures. Familiarity with home health practices, assisted living environments, group homes, nursing homes, or long-term care settings. Excellent communication skills for client interaction and team collaboration; understanding HIPAA privacy standards is essential. Ability to assist with transportation for appointments or community outings; experience supporting individuals with autism or intellectual disabilities is advantageous.
