Job Description
Job Summary We are seeking a compassionate and dedicated Direct Support Professional to provide essential support and care to individuals with varying needs. This role involves assisting clients in their daily activities, promoting their independence, and ensuring their overall well-being. The ideal candidate will have experience in home health care, patient monitoring, and a strong understanding of care plans tailored to individual needs. Responsibilities Overall responsibilities are to implement supports as outline in the individuals' support plan to accomplishment the desired outcomes. Provide services that are person-centered and encourage age appropriateness. Monitor and document client health status, reporting any changes or concerns to the appropriate healthcare professionals. Maintain agency operations to achieve compliance with contract requirements with Federal and State, Local regulations and any other licensing regulatory agencies as directed. Adhere to agency policy and procedures enhancing compliance with HIPPA requirements. Meet time lines in completing administrative tasks, and documentation Provide companionship and emotional support to clients, fostering a positive environment. Engage clients in social and recreational activities to enhance their quality of life. Maintain a weekly schedule for activity and events for clients. Requirements Previous experience in home health care or as a Direct Support Professional is preferred. Knowledge of patient observation techniques and ability to implement care plans effectively. Experience in senior care or working with individuals with dementia is highly desirable. Strong communication skills and the ability to build rapport with clients and their families. Ability to lift and assist clients as needed while ensuring their safety and comfort. Compassionate nature with a commitment to providing high-quality support and care. Must be flexible, resourceful, decisive, mature, tactful and professional and demonstrate leadership skills, the ability to multitask and be organized Certifications including CPR/First Aid is a plus but not mandatory upon hiring.
