Support residents with mobility and transfers using manual handling techniques and equipment like Hoyer lifts or patient transfer methods. Monitor patients' health by taking vital signs, observing for changes, and reporting findings in accordance with HIPAA regulations and care plans. Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary, ensuring safety during meal times. Help clients with medication administration following prescribed schedules and safety protocols. Maintain a clean, safe environment by performing light housekeeping duties including cleaning personal spaces and common areas. Provide transportation assistance for appointments or community outings, ensuring safety during travel. Document care activities accurately and communicate effectively Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly valued. Demonstrated ability to observe patients closely, monitor vital signs, and recognize signs of distress or changes in condition. Experience working with individuals with dementia Knowledge of care plans, medication administration procedures, and patient safety protocols such as CPR and first aid certification is essential. Familiarity with patient transfer methods including Hoyer lifts or other assistive devices enhances your effectiveness in this role. Strong communication skills for client interaction and collaboration within multidisciplinary teams are crucial.
