Position Summary:

Project Impact Recovery & Wellness is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our multidisciplinary team in a new outpatient treatment facility serving individuals with substance use disorders. The LPN will play a key role in delivering high-quality nursing services within our Medication-Assisted Treatment (MAT) and Intensive Outpatient Program (IOP). Under the supervision of medical providers, the LPN will be responsible for medication administration, patient monitoring, supportive counseling, and coordination of care. This position requires strong clinical judgment, excellent communication skills, and a deep commitment to supporting individuals on their path to recovery.

Essential Duties and Responsibilities:

Provide direct nursing care to clients enrolled in MAT and IOP services. Administer medications, including methadone and buprenorphine, under provider supervision and in accordance with protocols. Monitor and document patient vital signs, withdrawal scores (e.g., COWS), and other relevant clinical data. Conduct phlebotomy and manage laboratory specimen collection and processing. Support medication-related activities including prior authorizations, prescription coordination, and state PMP (Prescription Monitoring Program) checks. Deliver patient education regarding medications, side effects, and treatment protocols. Provide brief supportive counseling and promote engagement in behavioral health services. Document all services in the Electronic Health Record (EHR) accurately and in a timely manner. Collaborate with the interdisciplinary team (providers, counselors, case managers) to support individualized treatment plans. Participate in crisis prevention and intervention using appropriate de-escalation techniques. Engage in performance improvement, quality assurance, and utilization review activities as required. Participate in flexible scheduling and on-call rotation as needed.

Minimum Qualifications:

Current and valid LPN license issued by the Arizona State Board of Nursing. Associate Degree in Nursing from an accredited institution required; Bachelor's Degree in Nursing preferred. Minimum of 6 to 12 months of LPN experience; preference given to candidates with experience in behavioral health, addiction medicine, or MAT programs. Knowledge of outpatient detox protocols and co-occurring disorders strongly preferred. Proficient in documentation using Electronic Health Records (EHR). CPR certification required (or must be obtained upon hire). Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire. TB Test clearance required at the time of hire and annually. Ability to work independently and collaboratively in a fast-paced environment.
