Qualifications:

Current RN/LPN license to practice professional nursing in the State of Arizona Minimum 1 year of recent experience (If less than one year, we can assist with training) Successfully demonstrate educational knowledge Willingness to work hours and shifts as necessary to provide safe patient care Current PPD or chest X-Ray Current CPR certification Reliable transportation Ability to work independently LPNs administer nursing care for patients in-home which include: Follows nursing care plan as initiated by RN Case Manager in the nursing care of patient. Monitors patient status with vital signs included during shift/visit and PRN. Report any changes to RN Case Manager and document status and/or care provided. Accurately administer and document all medications given. Accurately and completely complete documentation of the shift or visit on nursing notes. RNs administer nursing care for patients in-home which include: Utilizing the nursing process plan, coordinate, implement and evaluate the nursing care of he patient.

Job Type:

Full-time Maintain accurate and complete records of all care delivered. Assess patient status, including vitals, and report changes to RN Case Manager and document on nurses notes. Carry out MD orders accurately and effectively. Provide timely and accurate communication between patient's physician and RN Case Manager. Accurately administer and document all medications given.
