Position Overview:

Responsible for the development and implementation of home care services for PACE program participants. Coordinates contracted home care services, durable medical equipment (DME), incontinence care, and nursing home services. Assesses the home care needs of frail and elderly participants and develops individualized plans of care. Serves as a liaison between participants, families, caregivers, home care vendors, providers, and the Interdisciplinary Team (IDT). Participates in scheduled IDT, Care Coordination, and staff meetings. Promotes participant dignity, independence, self-determination, confidentiality, and access to appropriate care.

Key Responsibilities:

Assess participant needs using the nursing process and develop appropriate home care plans. Assess and coordinate DME and incontinence care needs in collaboration with the IDT. Follow up on participant care needs as directed by the provider. Collaborate with the IDT to evaluate and re-evaluate caregiver hours. Educate participants, families, and caregivers regarding approved caregiver hours and assignment plans. Participate actively in IDT meetings and contribute to participant care planning. Communicate changes in participant condition or needs to appropriate members of the care team. Coordinate home care services and monitor services provided by contracted vendors. Maintain accurate and timely documentation according to PACE departmental guidelines. Maintain participant confidentiality and protect sensitive information. Ensure compliance with applicable federal, state, and local regulations, as well as organizational policies and procedures. Perform other related duties as assigned. Required Qualifications Graduate of an accredited Vocational Nursing program. Current California LVN license issued by the California Board of Vocational Nursing and Psychiatric Technicians. Current CPR/BLS certification. At least 1 year of experience working with a frail or elderly population . Experience and knowledge of the physical, mental, and social needs of patients is highly desirable. Valid California Driver's License with a good driving record.

Skills & Abilities:

Bilingual skills preferred based on location: English/Spanish English/Mandarin English/Cantonese Compassionate, patient, and caring approach when working with elderly participants. Ability to work effectively as part of an interdisciplinary team. Ability to treat and care for geriatric populations. Ability to work within the scope of LVN licensure and professional authority. Strong communication and interpersonal skills. Ability to advocate for and promote participant rights, dignity, independence, and confidentiality. Strong organization, documentation, and care coordination skills.

Pay:

$30.00 - $35.00 per hour
