DOE What You'll Do:
The Impact You'll Make As a Licensed Vocational Nurse (LVN) , you will provide nursing care and service coordination to adults with complex medical and mental health needs, including individuals experiencing homelessness and substance use disorders.
Medical Care & Coordination Provide Nursing Services:
Deliver basic nursing services, including injections, medication administration, and health monitoring.
Coordinate Medical Care:
Coordinate and schedule primary care and specialty medical appointments to support continuity of care.
Support Medication Management:
Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services from The People Concern. Coordinate with
Pharmacies:
Work with pharmacies to ensure the timely delivery of client medications.
Educate Clients:
Provide patient education and supportive services to increase compliance with prescribed medication regimens.
Collaboration & Care Integration Deliver Integrated Care:
Work as part of an interdisciplinary team to provide comprehensive care to clients with co-occurring medical, mental health, and substance use needs.
Promote Continuity of Care:
Communicate effectively with internal staff and external healthcare providers to coordinate services and address client needs. Respond to
Crises:
Provide crisis intervention and triage clients to emergency services, including police, paramedics, or the fire department, when necessary. Participate in
Program Activities:
Attend weekly staff meetings and individual supervision sessions.
Documentation & Compliance Maintain Accurate Records:
Document all services accurately and in real time using the designated electronic health record or charting system.
Support Healthcare Navigation:
Maintain knowledge of Medi-Cal benefits and assist clients in navigating the healthcare system.
Protect Confidentiality:
Maintain compliance with HIPAA requirements and all agency confidentiality and nondisclosure standards.
Support Program Operations:
Perform additional tasks as assigned by the Director to support client care and program operations.
What You'll Bring:
Your Qualifications Education and Experience Current and unrestricted California Licensed Vocational Nurse license required . Experience in health sciences, social services, mental health, or a related field. One to three (1-3) years of clinical experience working with individuals with serious mental illness and/or other complex needs is preferred. Current CPR certification is preferred. Military-experienced candidates are strongly encouraged to apply. Skills and Abilities Strong understanding of confidentiality and nondisclosure requirements under HIPAA. Familiarity with Electronic Health Records (EHR) and other technology tools. Ability to perform effectively under circumstances of emotional stress or conflict , including when working with uncooperative clients. Strong written and verbal communication, organizational, documentation, and interpersonal skills. Ability to collaborate effectively with interdisciplinary teams and external healthcare providers. Valid California Driver's License, clean driving record, and insurability under Alcott's automobile insurance policy required. Successful Live Scan clearance required. Why Join Alcott?
