Job Overview We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) for Adult Patient to join our healthcare team. One on One Patient Care. Private Duty Nursing. family members will do showering most of the time. Bedbound patient, bed bath at least 2 to 3 times a week. Patient needs diaper change at least 1-2 times a day. Patient has dementia. The LVN will be responsible for providing high-quality nursing services, assisting with patient assessments, administering medications, and supporting overall patient well-being. LVN will have the supervision of RN Case Manager. Duties assessments by monitoring vital signs, evaluating physical conditions, and documenting medical histories using electronic health record (EHR) systems such as AXXESS. Assist with G-tube feeding, 110 lbs. has few diapaer change within a day. Support patient mobility and safety through proper use of equipment like Hoyer lifts and ensuring adherence to HIPAA regulations to protect patient confidentiality. Provide specialized care for patients with conditions such as Alzheimer's disease, dementia.. Educate patients and their families on health management strategies, discharge planning, medication administration, and lifestyle modifications to promote recovery and wellness.

Pay:

$28.00 - $30.00 per hour
