Role:
San Fernando Medical Center is looking for a talented and compassionate Licensed Physician Assistant or Family Nurse Practitioner to join our dedicated healthcare team in San Fernando, CA. Responsibilities:
Conduct comprehensive patient evaluations, including history-taking and physical examinations Diagnose and treat acute and chronic medical conditions across diverse patient populations Develop and implement individualized treatment plans and care management strategies Order and interpret diagnostic tests, labs, and imaging studies Prescribe medications and manage ongoing pharmacological therapies Collaborate with physicians, specialists, and care teams to ensure continuity of care Educate patients and families on health maintenance, disease prevention, and treatment options Maintain accurate and thorough clinical documentation in the EMR system
Requirements:
Active California license as a Physician Assistant (PA-C) or Family Nurse Practitioner (FNP) Current DEA certification and prescriptive authority Experience in primary care, family medicine, or a related clinical setting preferred Strong clinical assessment, diagnostic, and critical thinking skills Proficiency with electronic medical records (EMR) systems Excellent communication and interpersonal skills with patients and care teams Bilingual (English/Spanish) a plus given the local patient population
