LVN MDS Nurse (MDS Coordinator) 📍 Marin Post Acute | San Rafael, CA 💰 $43-$50/hour DOE 🕒 Full-Time Join Marin Post Acute as Our Next LVN MDS Nurse! Are you an experienced LVN with MDS, PDPM, and skilled nursing experience looking to take the next step in your career? Marin Post Acute is seeking a dedicated Full-Time LVN MDS Coordinator to join our clinical leadership team. In this essential role, you'll oversee the MDS process, support regulatory compliance, optimize reimbursement, and collaborate with our interdisciplinary team to ensure exceptional resident care and outcomes. If you're detail-oriented, organized, and passionate about quality care, we'd love to meet you! Responsibilities Coordinate and manage the MDS/RAI process in accordance with CMS, federal, and state regulations Complete accurate and timely MDS assessments and supporting documentation Develop and update comprehensive resident care plans Collaborate with nursing, therapy, dietary, and interdisciplinary team members Monitor PDPM documentation and reimbursement accuracy Track quality measures and identify opportunities for clinical improvement Assist with survey preparation, audits, and regulatory compliance initiatives Educate and support staff regarding MDS processes and documentation requirements Partner with facility leadership to ensure clinical and operational excellence
Qualifications Required:
Current, active California LVN license Minimum 1 year of MDS experience in a Skilled Nursing Facility (SNF) or Post-Acute setting Knowledge of
MDS 3.0, RAI
process, PDPM, Medicare, and Medicaid reimbursement Strong assessment, documentation, and organizational skills Ability to work independently while collaborating with interdisciplinary teams
Preferred:
