Per Visit / Contract Position Summary We are seeking a reliable and compassionate Licensed Vocational Nurse (LVN) to provide follow-up skilled nursing visits to patients in their homes. The LVN will monitor patients' conditions, provide nursing care as ordered, reinforce the plan of care, and communicate changes to the supervising nurse and healthcare team. Responsibilities Conduct scheduled follow-up nursing visits in patients' homes. Assess and monitor the patient's condition and vital signs. Provide nursing care according to the physician's orders and established plan of care. Perform medication review and reinforce medication instructions as appropriate. Provide wound care and other skilled nursing services within LVN scope of practice. Educate patients and caregivers regarding disease management, medications, safety, and care plans. Identify changes in the patient's condition and promptly report concerns to the supervising RN/physician. Complete accurate and timely visit documentation and required clinical records. Maintain patient confidentiality and follow agency policies and applicable regulations. Coordinate with the RN, physician, therapists, caregivers, and other members of the healthcare team as needed. Qualifications Current, active LVN/LPN license in the state of practice. Current CPR/BLS certification preferred or required according to agency policy. Previous home health experience preferred. Strong assessment, communication, and documentation skills. Reliable transportation and ability to travel to patients' homes. Ability to work independently while following the established plan of care. Compensation $50 per completed follow-up visit. Mileage reimbursement, supplies, scheduling expectations, and documentation requirements may be addressed separately according to agency policy and applicable regulations.Pay:
