Position Summary The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes on an intermittent basis under the direction and supervision of a Registered Nurse (RN). Care is delivered in accordance with physician orders, individualized care plans, and agency policies and procedures. Responsibilities Provide skilled nursing services according to physician orders and patient care plans. Administer medications and treatments as prescribed. Monitor patient conditions and promptly report changes to the RN and physician. Assist patients and families with education regarding disease management, medications, and self-care techniques. Maintain accurate, timely, and complete clinical documentation. Collaborate with physicians, RNs, therapists, and other healthcare professionals to coordinate patient care. Participate in patient care conferences and interdisciplinary team meetings. Support preventive, restorative, and rehabilitative nursing interventions. Follow infection control and aseptic techniques. Assist with medical record reviews, quality improvement initiatives, and agency compliance activities. Respond to on-call assignments, including nights, weekends, and holidays as scheduled. Ensure patient confidentiality and compliance with HIPAA regulations. Perform other duties as assigned. Qualifications Required Graduate of an accredited Practical Nursing program. Current and unrestricted LPN license in the state of practice. Current CPR certification. Strong clinical assessment and communication skills. Ability to work independently while managing multiple priorities. Reliable transportation and valid drivers license.
