at The Lorelton in Wilmington, Delaware, United States Job Description LPN(PRN) Obtains updated information on each resident's health status via verbal report and the Daily Log. Sign, and punch time card, obtain and sign for pager. Obtain med keys from off going nurse. Provides report to incoming staff, including sufficient information for follow-up action if necessary. Identifies safety hazards and initiate corrective action immediately. Identifies resident problems and emergency situations and initiate immediate "lifesaving" measures for all residents. Provides document, assess and evaluate resident care provided. Reports changes in resident's condition to physician, family or designated contact person and staff members. Administers and/or supervises medications. Assesses the effectiveness, interactions and reactions to medications and/or treatments. Performs treatments and/or procedures as ordered by the physician. Updates any information on the resident's profile sheet, as necessary. Completes quarterly summaries per schedule. Participates in development of policies and procedures. Participates in resident care conferences as needed. Performs all procedures using standard precautions procedures. Promotes a harmonious and safe environment for staff and residents. In collaboration with the Director of Nursing, will assess physical and mental changes of residents and coordinate the delivery of quality health care to all residents. Participates in monthly review and update of Physician's Order Forms (P.O.S.) and Medication Administration Records (M.A.R.s) Assists with meals and resident care. Reviews resident's charts monthly, following established protocol. Checks monthly to assure first aid kits are in the facility and are adequately stocked. Monitors supplies for Nursing Department and re-orders before depleted. Maintains new admission packets. Maintains resident and staff confidentiality at all times. Gives report of any pertinent information about resident conditions or staffing concerns. Provides direct supervision of the Certified Nursing Assistants (C.N.A.s) on duty. Functions as facility Manager when Administrative staff is not in the facility. Notifies the Administrator or Director of Nursing with any problems not easily resolved, or injury or illness of residents requires ER or hospitalization. Attends Department and General Staff meetings as scheduled. Assists with orientation of new staff. Provides ongoing in-servicing of staff in collaboration with the Director of Nursing. Prepares and conducts written employee performance evaluations of assigned staff. Reviews all documentation completed by the assigned C.N.A. on duty, checks for accuracy and compliance. Intervenes when necessary to assure that services are given as identified in the Personalized Service Agreement. Serves on-call for consultation and/or staffing relief on a rotating schedule.
