Job Summary We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our dynamic healthcare team in. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Records (EHR) systems such as Epic or Cerner. Administer medications and injections safely, including IV infusions and catheterizations, following strict aseptic techniques and medication protocols. Manage complex care needs such as Alzheimer's disease, dementia, memory care, hospice care, and acute pain management for diverse patient populations. Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices. Provide emergency response support in urgent situations like airway management, ventilator operation, or trauma care within Level I or Level II trauma center environments. Assist with diagnostic evaluations such as specimen collection/processing and medical imaging coordination while ensuring compliance with infection control policies. Educate patients and families on health coaching topics like medication administration, nutrition, discharge planning, and caregiver communication. Document all patient interactions accurately in EMR/EHR systems while maintaining strict adherence to HIPAA and clinical confidentiality policies. Experience Proven experience working with geriatric patients in skilled nursing facilities or assisted living communities is highly preferred. Prior experience with medical-surgical units or home & community care environments enhances your ability to deliver comprehensive patient care. Knowledge of infectious disease management, hospice services, trauma response protocols, and acute care procedures is desirable.
