Pay:
$45-$85/hour Supportive clinical team Meaningful one-on-one patient care in the home setting Responsibilities Participates in ongoing interdisciplinary assessment of the patient. Completes initial and ongoing comprehensive assessments of the patient's needs at appropriate time points as assigned. Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s). Contributes to the patient's plan of care/service plan. Administers medications, injections, and treatments following established protocols and safety guidelines. Monitors vital signs, performs physical examinations, and evaluates patient responses to interventions. Utilizes EMR/EHR systems to document patient information accurately and efficiently. Provides education on health management and medication administration to patients and their families. Collaborates with interdisciplinary teams including physicians, therapists, and case managers to ensure coordinated care delivery. Maintains strict adherence to HIPAA regulations to protect patient confidentiality and privacy. Assists with specialized procedures such as IV infusion, suturing, catheterization, and sterile processing when required. Uses clinical decision-making to efficiently and effectively manage individual patients. Participates in Quality Assessment and Performance Improvement (QAPI) program. Participates in in-services or training, as requested. Qualifications Valid RN or LPN license with current registration in the state of Florida Proven experience in acute care settings such as ICU or Level I Trauma Centers is highly desirable Experience working with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities, or those requiring hospice & palliative care Proficiency in using EMR/EHR systems such as Cerner or Axxess for documentation purposes 1 year of OASIS home health experience AHCA Level II Background Screening required Excellent communication skills coupled with a compassionate approach to patient care Experience Home health: 1 year (Required) License/Certification CPR Certification (Required) Florida RN or LPN License (Required) Valid Driver's License (Required) Join our team to make a meaningful difference by providing expert nursing care that promotes health, safety, and dignity for our clients in their homes!
