Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories, ensuring accurate documentation in electronic health records (EHR/EHR systems). Administer medications and injections safely, including IV infusions and catheterizations, following strict protocols and HIPAA confidentiality policies. Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging coordination to support accurate diagnoses. Manage airway care, ventilator support, tube feeding, and wound care procedures while maintaining aseptic techniques. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral health management. Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care to ensure seamless patient transitions. Educate patients and caregivers on medication administration, health coaching, and chronic condition management to promote self-care and wellness. Experience Proven experience working as an LPN in diverse healthcare settings such as hospitals (including ICU or Level I Trauma Center), nursing homes, skilled nursing facilities, or home & community care environments. Demonstrated proficiency with EMR/EHR systems like Cerner or similar platforms for accurate medical documentation. Critical care experience including ICU or emergency medicine is highly desirable; familiarity with Level II trauma centers or Level I trauma center protocols is a plus. Knowledge of ICD coding (ICD-9/ICD-10) for medical records documentation and billing purposes. Experience working with geriatric patients or individuals with disabilities—especially those requiring dementia or memory care—will be valued. Skilled in performing procedures such as phlebotomy, suturing, sterile processing techniques, and first aid. Strong understanding of long-term care regulations, infection control practices, and physical examination techniques to ensure compliance and safety standards are met.
