Job Summary Von & Bick Healthcare Associates, LLC is seeking a dependable, compassionate, and experienced Licensed Practical Nurse (LPN) to provide one-on-one support to an adult male in his home in Lawrenceville, Georgia. The ideal candidate will be patient, friendly, attentive, and comfortable supporting an individual with significant physical and medical support needs. The individual has no known behavioral concerns and communicates primarily through sounds and gestures. The individual requires assistance with G-tube care, feeding and hydration, mobility/transfers using a Hoyer lift, personal care, and other health-related supports as needed. Duties Providing G-tube-related care and support Assisting with feedings and hydration Assist with and/or administer medications, including medications via G-tube when required, in accordance with physician orders, the individual's healthcare plan, and applicable nursing requirements Assisting with transfers using a Hoyer lift Positioning and mobility assistance Supporting personal care and the evening/bedtime routine as assigned Observing and reporting changes in health or condition Following the individual's approved service plan, healthcare plan, physician orders, and safety protocols Maintaining accurate and timely documentation Communicating appropriately with the family and Von & Bick clinical/administrative team Schedule Approximately 13 hours per week

•9:00 PM Weekend availability is required. Additional hours may occasionally be available based on the individual's authorized services and family needs. Qualifications Current, unrestricted Georgia LPN license Comfortable and experienced with G-tube care Comfortable using a Hoyer lift and assisting with transfers Ability to safely lift 25+ pounds Ability to provide medication, feeding, hydration, mobility, and personal-care support as authorized Current CPR/First Aid certification Dependable transportation Strong communication and documentation skills Compassionate, patient, and professional demeanor Ability to successfully complete all required background screening, credentialing, and onboarding requirements IDD experience preferred, but not required How to Apply If you are a compassionate, dependable LPN who enjoys providing personalized one-on-one care and helping individuals remain safe and supported in their home and community, we would love to hear from you.
