LPN || PRN || All Shifts Chapters Living of Council Bluffs - 3.0 Council Bluffs, IA Job Details PRN 1 day ago Qualifications CPR Certification Resident personal care assistance Care documentation Assisting with ADLs LPN Body mechanics Full Job Description Essential Duties and Responsibilities for Licensed Practical Nurses (LPNs):
- Provide direct resident care services. Follow individualized plan of care for all residents.
- Administer medications following all standards of practice, state regulations, and policies to include all required documentation, reporting and follow up.
- Perform resident assessments as required by the state standards/ regulations and facility policy, under the direction of the Director of Nursing.
- Observe residents for change in condition and report any changes immediately to supervisor and/or the Director of Nursing
- Document all pertinent/relevant observations, communication with the resident's responsible party, clinical procedures and daily assistance/care by the end of each shift.
- Complete and maintain current individualized care plan for each resident.
- Check and record vital signs, weights, and blood sugars as assigned.
- Provide for or assist residents with activities of daily living as needed, including bathing/showing according to schedule, oral care, toileting, dressing, etc. Encourage residents to participate in scheduled facility activities providing assistance in getting to activities as needed. Assist in the planning and implementation of resident activities as needed.
- Assist residents with transfers and ambulation as directed, using proper body mechanics.
- Supervise direct care staff ensuring that resident care and medication assistance is provided correctly and in accordance with policies and state regulations.
