Job Overview We are seeking a dedicated and compassionate 2ND SHIFT Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our healthcare team. Schedule Monday-Thursday 2-10:30pm Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems. Administer medications and injections accurately. Monitor patients' conditions closely, managing acute pain and responding promptly to changes in status. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Assist with diagnostic evaluations, specimen collection, and processing to support accurate clinical decision-making. Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and patient information. Provide education and support to patients and their families regarding treatment plans, medication administration, and discharge instructions. Maintain sterile techniques during procedures such as catheterization and adhering to infection control policies. Document all patient interactions thoroughly in EMR/EHR systems, ensuring accuracy and completeness of medical documentation. Support the care of geriatric patients, including those with dementia or Alzheimer's disease, within long-term care or assisted living settings. Experience Valid RN or LPN license with current state licensure required; additional certifications such as BLS/CPR are essential. Prior experience working in long-term care facilities Experience with EMR/EHR systems Proven ability to handle complex cases involving acute pain management, infectious disease care, or hospice services.
