Company Overview:
AAA Hoosier Home Healthcare is a leading provider of in-home health services committed to delivering compassionate, high-quality care to patients in the comfort of their homes. We are currently seeking dedicated and skilled Licensed Practical Nurses (LPNs) to join our growing team and provide care to a patient located in Schererville, Indiana.
Job Summary:
The LPN will deliver direct nursing care in accordance with physician orders and established agency policies. Responsibilities include using OASIS for patient documentation, coordinating care plans, and working collaboratively with RNs, HHA's, and other team members to ensure optimal patient outcomes.
Key Responsibilities:
Perform initial home health visits and ongoing patient assessments Complete admissions, transfers, resumptions of care, re-certifications, and discharges using OASIS Develop and implement individualized care plans under the direction of a physician Educate patients and caregivers on medications, treatments, disease processes, and home care procedures Monitor patient condition, document observations, and notify the appropriate team members of any changes Coordinate care with interdisciplinary team members including RNs, therapists, and case managers Ensure timely and accurate documentation in clinical records Attend and participate in case conferences for care coordination Adhere to agency policies and maintain compliance with Medicare/Medicaid regulations Participate in ongoing professional development and training
Qualifications:
Graduate of an accredited LPN program Current LPN license in the state of Indiana Minimum of one year of nursing experience (home health preferred) Proficiency with OASIS documentation is highly desirable Strong communication and organizational skills Ability to work positively and collaboratively with patients, families, and team members
