Job Description

Sevita is seeking a compassionate LPN to provide person-centered care in home and community-based settings. The Licensed Practical Nurse will deliver direct nursing care, administer medications, monitor vital signs, and support individuals with diverse medical and behavioral needs. In this role, you'll collaborate with RNs, therapists, and caregivers to implement care plans, document in EHR systems, and ensure safety and compliance with clinical standards. Responsibilities Provide direct nursing care in home and community-based settings Administer medications and treatments per provider orders Monitor and document vital signs and patient status Assist with wound care and basic clinical procedures Implement and update individualized care plans with the clinical team Accurately document care in electronic health records (EHR) Educate individuals and families on health conditions and care routines Ensure compliance with HIPAA, safety, and infection control standards Collaborate with RNs, therapists, and caregivers to coordinate services Participate in ongoing training and professional development offered by Sevita Required Skills Vital signs monitoring Medication administration Wound care Care planning documentation Patient assessment EHR documentation Infection control practices IV therapy (where permitted) Care coordination Patient and family education
