00 PER VISIT. For routine skilled nursing visits, some experienced nurses are able to complete approximately two patient visits per hour. More comprehensive visit types (such as OASIS assessments) may require additional time. Actual earnings may vary depending on the nurse's determined pay rate and visit type. Job Overview:
We are seeking a compassionate and dedicated Home Care Nurses (RN/LPN) to provide high-quality, patient-centered care in patients' homes throughout the assigned service area. The ideal candidate will possess strong clinical, communication, and organizational skills and be committed to delivering exceptional care to patients and their families.
Key Responsibilities:
Provide skilled nursing care to patients in their homes in accordance with physician orders and agency policies. Assess patient needs, monitor clinical status, and document visits accurately and timely. Educate patients and families regarding diagnoses, medications, treatments, and care plans. Collaborate with physicians, caregivers, and interdisciplinary team members to ensure quality patient outcomes. Participate in care planning, case coordination, and ongoing communication regarding patient progress. Maintain compliance with agency policies, state regulations, and professional nursing standards.
Qualifications:
Current Massachusetts RN or LPN license in good standing in Massachusetts. Minimum one (1) year of nursing experience; home care experience preferred. Current BLS certification. Valid driver's license and reliable transportation. Strong communication, organizational, and documentation skills. Must have a criminal background check Responsibilities will be assigned in accordance with the scope of practice of the candidate's licensure (RN or LPN).
