The primary responsibility of this position is medication management , along with routine health monitoring, patient support, and communication. The nurse will visit the patient daily, communicate with her in Spanish, review and manage medications according to the physician's orders, monitor the patient's response to medications, and identify and report any changes in her condition. The ideal candidate should be compassionate, reliable, professional, and experienced in medication management and home-based patient care. Strong Spanish communication skills are required to ensure the patient clearly understands her medications and care instructions. Responsibilities Provide daily nursing visits in the patient's home. Communicate with the patient in Spanish and provide clear, compassionate support. Manage and monitor the patient's medications according to the physician's orders and care plan. Review medication names, dosages, schedules, and instructions with the patient. Assist with medication administration as permitted by the nurse's scope of practice. Monitor medication adherence and identify missed, incorrect, or refused doses. Observe for medication side effects, adverse reactions, or other concerns. Monitor the patient's vital signs and overall health status as appropriate. Assess for changes in the patient's physical, mental, or emotional condition. Provide patient education regarding medications and treatment instructions. Communicate significant changes or concerns to the physician or supervising healthcare professional. Maintain accurate and timely documentation of each daily nursing visit. Provide reassurance, companionship, and emotional support while maintaining professional boundaries. Promote patient safety and identify potential risks in the home. Follow all physician orders, nursing standards, and the established plan of care. Requirements Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN) license , as required for the position and applicable state regulations. Experience with medication management and medication administration . Previous experience providing home health, visiting nurse, or private-duty nursing care preferred. Fluent Spanish-speaking ability required for effective communication with the patient. Female nurse preferred/required , based on the patient's stated care preference. Strong knowledge of medication safety, dosage schedules, and patient monitoring. Ability to recognize and promptly report changes in the patient's condition. Excellent communication and interpersonal skills. Compassionate, patient, dependable, and professional demeanor. Ability to work independently during home visits while following the physician's orders and plan of care. Strong documentation and organizational skills. Ability to maintain patient confidentiality and comply with applicable privacy and healthcare regulations. Current CPR/BLS certification preferred or required according to the employer's policy. Reliable transportation for daily visits, if required by the position. Must successfully complete any required background checks, credential verification, and employment requirements.
