Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to patients across various settings, including long-term care facilities, assisted living communities, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and patient education. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Monitor patients' conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns appropriately. Assist with medical procedures such as wound suturing, specimen collection, and respiratory support including ventilator management. Maintain detailed medical records using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA and confidentiality policies. Provide patient education on medication management, disease processes such as dementia or Alzheimer's care, and post-discharge instructions. Support discharge planning by coordinating with multidisciplinary teams to ensure seamless transitions of care. Implement infection control practices diligently to prevent healthcare-associated infections. Collaborate effectively with physicians, case managers, and caregivers to optimize patient outcomes. Demonstrate proficiency in specialized areas such as geriatrics, pediatrics, hospice care, or trauma center environments when applicable. Skills Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, or those requiring memory or dementia care. Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Strong knowledge of medical terminology, anatomy, physiology, and pharmacology related to nursing practice. Skilled in medication administration techniques including injections, venipuncture, and IV insertions. Ability to perform physical examinations and vital sign assessments accurately. Familiarity with infection control protocols and aseptic techniques. Excellent communication skills for effective caregiver interaction and client education. Experience in specialized areas such as ICU, emergency medicine, long-term care regulations, or hospice is highly desirable. Knowledge of ICD coding (ICD-9/10), CPT coding for procedures when applicable. Capable of working efficiently in fast-paced environments such as emergency rooms or trauma centers while maintaining attention to detail.
