Duties Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations Administer medications, injections Perform catheterization, catheter care Educate patients on health coaching topics including nutrition and chronic disease management Ensure infection control practices are maintained in all procedures Maintain compliance with HIPAA regulations and medical documentation standards Qualifications Valid Licensed Practical Nurse (LPN) license in the state of practice Proven experience in geriatrics (including Alzheimer's and dementia care) Medical terminology; and medical documentation standards Knowledge of anatomy and physiology relevant to patient assessment and diagnostic evaluation Ability to work independently as well as part of a multidisciplinary team in long term care setting Strong communication skills with the ability to work effectively with patients and individuals with disabilities or complex health needs Commitment to infection control protocols and aseptic techniques BLS certification required This role is ideal for licensed practical nurses committed to delivering exceptional patient
