Job Description:
Hospice LPN provides support to patients and families inquiring about hospice care and assists with the intake process when they are referred to our hospice program. Communicates with referral sources, physicians, and other health care professionals to ensure timely and accurate hospice enrollment.
Responsibilities:
Receives inquiries about hospice care and explains services and the enrollment process to patients and families. Coordinates and schedules patient visits with interdisciplinary team. Reviews and verifies patient eligibility and benefit coverage under Medicare, Medicaid, or private insurance. Assures that all necessary consent forms and documents are obtained and properly completed before initiation of care. Processes and maintains patient files and documentation. Communicates with physicians, referral sources, and other healthcare professionals to ensure accurate and timely enrollment into the hospice program. Provides daily updates to the Interdisciplinary Team about new patients, follow-up visits, and date-driven requirements. Weekly audits
Qualifications :
At least 2 years of experience in hospice care coordination. Must have excellent verbal and written communication skills. Ability to multitask and manage a heavy workload in a fast-paced environment. Knowledge of medical terminology and healthcare regulations. Strong computer skills, including proficiency in electronic health records and Microsoft Office. Must have the ability to maintain patient confidentiality. The Hospice LPN must be compassionate and empathetic with excellent active listening skills and the ability to work with people from diverse backgrounds with varied orientations to care at the end of life.
