Across six locations, we provide primary care, urgent care, specialty care, and occupational medicine. Through our joint venture with RWJBarnabas Health, we work to connect patients with coordinated care. We are looking for an LPN Care Coordinator who enjoys helping patients navigate their care and keeping communication moving among patients, families, and healthcare providers. In this role, you will work from our corporate office in Linden and support patients by telephone. You will help coordinate care, address questions and concerns, and communicate with the healthcare team so patients understand their next steps and receive appropriate follow-up. This is a good opportunity for an LPN who values patient relationships and wants to use their clinical knowledge in a care coordination role. The position does not involve hands-on patient care.
Responsibilities:
Reviewing refill requests and facilitating prescription refills in adherence to established protocols and guidelines. Thoroughly assessing and documenting normal imaging/ lab results, ensuring accurate record-keeping and appropriate follow-up actions as necessary. Maintaining comprehensive and up-to-date patient charts by documenting all pertinent information accurately and in a timely manner. Communicating important information to patients regarding their care plans, test results, and any necessary follow-up appointments or treatments.
Care Planning:
Develop individualized care plans in collaboration with healthcare teams to ensure that patients receive appropriate and coordinated care.
Education:
Educate patients and families about treatment plans, medications, and self-care strategies, ensuring they understand their conditions and how to manage them.
Care Coordination:
Coordinate and schedule appointments, tests, and referrals to specialists, ensuring continuity of care and timely interventions.
Monitoring:
Continuously monitor patients' progress and adjust care plans as needed to achieve optimal outcomes.
Advocacy:
Advocate for patients' needs and rights, ensuring they receive quality care and support throughout their healthcare journey.
Resource Management:
Assist patients in accessing community resources, financial assistance, and support services that can aid in their recovery and well-being.
Quality Improvement:
Collaborate with healthcare teams to improve the quality of care and patient satisfaction.
Compliance:
Ensure that care plans and interventions adhere to all relevant healthcare regulations and standards.
Case Management:
Some RN Care Coordinators may also be involved in managing complex cases, particularly those with chronic or multiple health conditions.
Care Planning:
Develop individualized care plans in collaboration with healthcare teams to ensure that patients receive appropriate and coordinated care.
Qualifications:
Valid and current LPN license in the state. Prior experience in a clinical setting, preferably in a care coordination or similar role. Familiarity with electronic health records (EHR) and other relevant healthcare software. Excellent organizational skills with the ability to prioritize tasks effectively in a fast-paced healthcare environment. Strong interpersonal and communication skills, demonstrating empathy and compassion in patient interactions.
Preferred Qualifications:
Additional certifications or training in care coordination or related healthcare fields. Proficiency in utilizing various healthcare management software and electronic medical records systems.
