RESPONSIBILITIES

We are seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN) to join our clinical team. You will deliver high-quality bedside care, administer medications, manage wound treatments, and coordinate with interdisciplinary healthcare teams to optimize resident recovery and long-term wellness.

Position Highlights Medication Administration & Treatment:

Administer prescribed medications, IV therapies (if certified), and treatments according to physician orders and state standards of care.

Resident Assessment & Monitoring:

Conduct daily clinical assessments, monitor vital signs, track change in condition, and report findings to the RN or attending physician.

Care Plan Execution:

Collaborate with physical therapists, occupational therapists, physicians, and Registered Nurses to execute personalized interdisciplinary care plans.

Documentation:

Maintain precise, timely, and compliant resident medical records using our electronic health record (EHR) system.

Resident & Family Communication:

Educate residents and their families on care plans, recovery goals, and discharge instructions with empathy and clarity.

Supervision:

Oversee Certified Nursing Assistants (CNAs) on shift, ensuring safe infection control practices, mobility assistance, and quality daily living care.

REQUIREMENTS

Must be a graduate of an approved school for practical or vocational nursing. Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure. Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
