RESPONSIBILITIES

Are you a Registered Nurse who thrives on building meaningful patient relationships while utilizing high-level clinical skills? Join our interdisciplinary team as a Direct Care RN in a skilled nursing and rehabilitation center. Role Overview As a Direct Care RN, you are the backbone of our post-acute rehabilitation and long-term care teams. You will manage clinical conditions, administer advanced therapies, and lead certified nursing assistants (CNAs) to deliver compassionate, evidence-based care.

Position Highlights Clinical Care Delivery:

Assess, plan, and execute comprehensive nursing care for short-term rehabilitation guests and long-term skilled nursing residents.

Sub-Acute Interventions:

Manage IV therapies, complex wound care, tracheostomy care, enteral feedings, and post-surgical recovery protocols.

Interdisciplinary Collaboration:

Work closely with physical, occupational, and speech therapists, physicians, nurse practitioners, case managers and other professionals to achieve patient rehabilitation goals.

Care Coordination:

Conduct continuous health assessments, update individual care plans, and lead resident status reviews.

Delegation & Leadership:

Supervise, mentor, and direct LPNs and CNAs to ensure safety, dignity, and high quality of care.

Documentation & Compliance:

Maintain accurate electronic health records (EHR) in compliance with state, federal, and MDS guidelines.

QUALIFICATIONS

Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing CPR Certification is required
