Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in a nursing home setting. As an LPN, you will play a critical role in supporting the health and well-being of our residents, ensuring they receive the best possible care. Responsibilities Provide direct patient care, including administering medications and treatments as prescribed by physicians. Conduct patient assessments and monitor vital signs to ensure optimal health outcomes. Assist with daily living activities and support patients with personal hygiene needs. Collaborate with healthcare teams to develop and implement individualized care plans. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Educate patients and their families about health management, medication administration, and treatment plans. Participate in triage and emergency response situations when required. Uphold infection control protocols and ensure a safe environment for patients and staff. Qualifications Valid LPN license in the state of practice. Experience in long-term care is preferred. Strong skills in patient assessment, medication administration, and basic life support techniques. Ability to work collaboratively within a multidisciplinary team while demonstrating excellent communication skills.
