As a Licensed Practical Nurse (LPN) with Attentive Care, you will play a vital role in delivering high-quality nursing care in long-term care and rehabilitation settings. Working collaboratively with Registered Nurses, Certified Nursing Assistants, physicians, and the interdisciplinary care team, you will provide skilled clinical care, monitor resident health, administer treatments and medications, and help ensure each resident receives safe, compassionate, and person-centered care. Responsibilities Provide direct nursing care to residents in accordance with individualized care plans, physician orders, and facility policies. Administer prescribed medications, treatments, and delegated nursing procedures accurately and safely while monitoring for effectiveness and adverse reactions. Assess, monitor, and document residents' vital signs, physical condition, and changes in health status, promptly reporting significant findings to the Registered Nurse or attending provider. Perform wound care, dressing changes, specimen collection, catheter care, and other nursing procedures within the LPN scope of practice. Maintain accurate, timely, and complete documentation in the electronic or paper medical record in accordance with regulatory and facility requirements. Supervise and support Certified Nursing Assistants (CNAs) by providing direction, monitoring delegated tasks, and promoting high standards of resident care. Collaborate with Registered Nurses, physicians, therapists, and the interdisciplinary care team to ensure coordinated, high-quality resident care. Respond appropriately to changes in resident condition, medical emergencies, and infection prevention protocols while following established clinical procedures. Promote resident dignity, comfort, safety, and independence while building positive relationships with residents, families, and fellow healthcare professionals. Adhere to all applicable state and federal regulations, facility policies, infection control standards, and patient confidentiality requirements. #IND123 Requirements Hold a current New York State LPN license. Recent facility-based nursing experience in a nursing home, rehab center, or assisted living setting, including recent facility staff relief experience. Active BLS or CPR certification. Eligibility to work in the United States. Reliable transportation and residence within 25 miles of the facility where you will be working. At least 6 months of facility-based nursing experience within the past year is a plus. Benefits Why Join Attentive Care?
