Job Responsibilities:

Provide direct nursing care to residents in a Long-Term Care and Skilled Nursing Facility setting. Administer medications and treatments according to physician orders and facility protocols. Monitor resident conditions, vital signs, symptoms, and changes in health status. Perform wound care and other nursing procedures within the LPN/LVN scope of practice. Assist with care planning and communicate changes in resident conditions to the appropriate healthcare team. Maintain accurate, timely, and complete nursing documentation. Collaborate with residents, families, physicians, RNs, CNAs, and interdisciplinary healthcare professionals. Follow infection control, resident safety, medication administration, and facility policies. Provide compassionate, resident-centered care while maintaining professional nursing standards.

Requirements:

Active and unrestricted New York LPN/LVN license . Minimum 1 year of recent LPN experience in Long-Term Care (LTC) required. Current BLS/CPR certification . Previous experience in LTC, SNF, nursing home, rehabilitation, post-acute care, or skilled nursing preferred. Strong clinical assessment, communication, organization, and time-management skills. Ability to work the assigned day/night schedule. Commitment to completing the full 13-week contract assignment.
