Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Administer medications, injections, IV infusions, and treatments such as tube feedings and catheter care with precision and adherence to safety protocols. Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures while maintaining sterile technique and infection control standards. Monitor patients' conditions closely, manage acute pain, airway management, ventilator support, and respond promptly to urgent or emergent situations such as trauma or cardiac events. Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care regulations to optimize patient outcomes across settings like skilled nursing facilities or hospice care. Maintain accurate medical documentation in compliance with HIPAA confidentiality policies and ICD coding standards (ICD-9/10) for billing and recordkeeping purposes. Provide education and support to patients and their families on health coaching, medication administration, dementia or Alzheimer's care, and other specialized needs like pediatric or neonatal care. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current CPR/BLS certification. Proven experience working with diverse populations including geriatrics, individuals with developmental disabilities, or those requiring memory or behavioral health support. Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks; familiarity with medical terminology and ICD coding is highly preferred. Critical care experience in settings like ICU or Level I Trauma Centers is advantageous; experience in emergency medicine or urgent care is a plus. Strong knowledge of anatomy, physiology, infection control practices, and long-term care regulations to ensure safe patient assessments and interventions. Excellent communication skills for caregiver collaboration and client education; ability to work independently while following clinical confidentiality policies. Flexibility to adapt to various clinical environments including hospital medicine, outpatient clinics, home & community care settings or travel nursing assignments.
