Job Description

LPN or Certified Medical Assistant - Clinical Support Team Full-Time | Onsite - Brookfield, WI Lily Medical Group is seeking a full-time LPN or Certified Medical Assistant to join our clinical support team at our Brookfield, WI office. This is a mandatory onsite position based at our Brookfield office. This position is not remote or hybrid, and regular in-person attendance at the Brookfield location is required. At Lily Medical Group, we provide compassionate, high-quality primary care services to seniors where they live, including assisted living, independent living, and other geriatric care settings. This position is a great fit for someone who is organized, dependable, and enjoys helping coordinate patient care while working closely with physicians, nurse practitioners, facilities, patients, families, pharmacies, and other healthcare partners. This role is not a traditional bedside nursing position. The Clinical Support Team Member will assist with clinical workflows, medication-related support, care coordination, documentation, communication, and follow-up needs to help ensure patients receive timely and high-quality care.
Responsibilities Include:
Supporting physicians and nurse practitioners with clinical and documentation workflows Assisting with medication reconciliation and medication-related follow-up Coordinating care between providers, facilities, patients, families, pharmacies, and healthcare partners Assisting with clinical triage and follow-up coordination Reviewing and entering documentation in the electronic health record Supporting provider orders, recommendations, and care plan updates Maintaining accurate and timely patient records Communicating professionally with internal team members and external care partners Traveling to local senior living and care facilities as needed
Qualifications:
Active Wisconsin LPN license or nationally certified Medical Assistant credential required Ability to work onsite at the Brookfield, WI office Monday through Friday Experience in geriatrics, assisted living, primary care, care coordination, or medication management preferred Strong communication, organization, and problem-solving skills Ability to work independently and as part of a team Comfortable using electronic health records and healthcare technology systems Valid driver's license and reliable transportation required
Schedule:
Full-time Monday through Friday Onsite at the Brookfield office No weekends No evenings No holidays
