Sterling Medical is looking for LPN's with Referral Management Experience for Virtual Healthcare Services for Tripler Army Medical Center, Honolulu, Hawaii

THIS IS A TEMPORARY POSITION FOR AN EMPLOYEE THAT IS ON MEDICAL LEAVE

This position is for Virtual Health within the military healthcare system-There is NO hands on patient care with this position! There would be some on-site training and after training is complete, the position would be remote or teleworking GTP is a vision for better healthcare for our military members in need of care globally This is a great opportunity for an LVN/LPN interested in a different way of healthcare- no hands-on patient care, but the opportunity to assist our military to get the help they need in a different kind of way through the military Virtual Health System or Global Teleconsultation Platform Competitive Compensation! PTO is accrued as a temporary employee and there are 11 Paid Federal Holidays!

Qualifications:
Education:

• Certificate from Licensed Practical Nursing program, LVN program from a college university or accredited LPN/LVN training program

Experience:

•One year of experience as an LVN or LPN Must have a background overseeing and coordinating patient care Referral Management Experience preferred

License:

Must have a current, unrestricted license to practice as a

Licensed Practical Nurse Duties:

Including, but not limited to: Maintain patient privacy and confidentiality, as outlined in the Health Insurance Portability and Accountability Act and Personally Identifiable Information. When Potential delays may compromise patient's condition, coordinate with appropriate medical authorities designated by the government staff to find alternative sources of care Oversee and monitor the review, coordination, and tracking of Virtual Healthcare referrals, urgent/ASAP referrals, and VA-DoD sharing referrals. Schedules referral appointments in accordance with required Access to Care Standards within the direct care system outside the MTF with network/non-network providers. Ensures appointments are done within the Access to Care standards for 90% of all referrals. Coordinates with specialty referral clinics (internal or external) to obtain special patient instructions and/or tests required prior to appointment. Provides pre-appointment instructions to patients as well as the details regarding their referral appointment (i.e., date/time, provider, and location). Ensures patients receive necessary documentation appropriate for the referral medical care visit.

Certification:
