Job Overview Join our dedicated healthcare team as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) specializing in Long Term Care. In this vital role, you will provide compassionate, comprehensive nursing care to residents in a long-term care setting, ensuring their comfort, safety, and well-being. You will be instrumental in delivering high-quality patient care through assessments, medication administration, and collaboration with interdisciplinary teams. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring ongoing support and medical attention. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to develop personalized care plans. Administer medications and treatments accurately, including injections, IV infusions, tube feedings, and wound suturing, adhering strictly to aseptic techniques and safety protocols. Monitor residents' conditions closely using telemetry and other monitoring systems; respond promptly to acute changes or emergencies such as trauma or critical illness. Document all patient interactions and treatments precisely within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with physicians and interdisciplinary teams on diagnostic evaluations, medication adjustments, discharge planning, and complex case management involving behavioral health or memory care needs. Provide specialized care for diverse populations including geriatrics, individuals with developmental disabilities, hospice patients, and those requiring trauma or infectious disease management. Support residents' activities of daily living (ADLs), including catheter care, ostomy management, and tube feeding while maintaining dignity and respect. Qualifications Current licensure as an RN or LPN in good standing; additional certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Pediatric Advanced Life Support (PALS) are preferred. Proven experience working in long-term care facilities or nursing homes; experience with memory care or behavioral health is highly desirable. Familiarity with electronic health records (EHR) systems such as Epic, Cerner, Athenahealth or eClinicalWorks; proficiency in medical documentation and ICD coding (ICD-10/ICD-9). Strong knowledge of medical terminology, physiology, aseptic techniques, infection control practices, and medication administration protocols. Ability to perform complex patient assessments including vital signs monitoring and laboratory specimen collection/processing. Experience working with individuals with disabilities or chronic conditions requiring specialized interventions like dialysis or ventilator management is advantageous.
