This dynamic role offers flexibility and the opportunity to provide specialized end-of-life care in a supportive environment. Duties Perform comprehensive patient assessments, including vital signs, symptom management, and overall health evaluations to develop personalized care plans. Administer medications, including IV infusions, injections, and other treatments following strict protocols and safety standards. Provide wound care, catheterization, tube feeding, and other skilled nursing procedures with precision and compassion. Monitor patient conditions closely, recognizing signs of acute pain or distress and implementing appropriate interventions promptly. Collaborate with interdisciplinary teams using EMR/EHR systems such as Epic or Cerner to document patient care accurately and maintain medical records in compliance with HIPAA regulations. Educate patients and families about disease processes like Alzheimer's, dementia, or infectious diseases, fostering understanding and comfort. Support patients with complex needs such as ventilator management, oxygen therapy, or palliative symptom control in various settings including home care or skilled nursing facilities. Experience Valid LVN license with current state licensure required. Prior experience working in hospice or palliative care settings is highly desirable. Proficiency with EMR/EHR systems such as Epic or Athenahealth for accurate documentation. Experience in long-term care environments like nursing homes or assisted living facilities is beneficial. Strong communication skills to effectively coordinate with caregivers, families, and healthcare teams while maintaining clinical confidentiality policies.
