Overview We are seeking a dedicated and highly skilled Licensed Vocational Nurse (LVN) to serve as a Field Nurse, providing essential healthcare services directly within the community. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems. Administer medications via injections, IV infusions, and other routes following strict aseptic techniques and medical protocols. Provide wound care, catheterization, tube feeding management, and ventilator support as required for patients with complex needs. Perform specimen collection and processing for laboratory testing while maintaining specimen integrity and adhering to infection control standards. Educate patients and caregivers on disease management, medication administration, nutrition, and self-care strategies to promote independence. Coordinate with healthcare providers to develop individualized care plans that align with long-term care regulations and best practices. Ensure compliance with HIPAA privacy regulations and maintain accurate medical documentation within EMR/EHR systems. Manage urgent or emergent situations through triage protocols, first aid procedures, airway management, and basic life support (BLS). Experience Valid LVN license with current registration in the state of practice; additional certification in BLS/CPR required. Demonstrated experience working in diverse healthcare settings such as home care, skilled nursing facilities, hospitals (including ICU or trauma centers), or assisted living communities. Proven proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient management. Critical care experience including ICU or Level I/II trauma center exposure is highly desirable. Experience working with individuals with developmental disabilities, dementia care (including Alzheimer's), memory care patients, or those requiring hospice services. Knowledge of

coding systems for accurate medical records documentation. Familiarity with infection control practices, sterile processing techniques, and post-acute care procedures.
