North Dallas, Carrollton, Addison, Farmers Branch, Garland, Rowlett, Rockwall, and surrounding areas. Schedule & Territory Monday through Friday 8-hour day shifts 40 hours per week Coverage territories include: North Dallas Carrollton Addison Farmers Branch Garland Rowlett Rockwall Surrounding communities May be required to assist in other service areas within approximately a 60-mile radius of the facility. Productivity Requirements LVNs are required to achieve 32 productivity points per week . Routine home health visits are valued at 0.9 points per visit . Required Qualifications Active LVN/LPN license. Recent Home Health experience (2024-2025 preferred/required). Experience managing a home health caseload and completing routine visits. Reliable transportation and ability to travel throughout assigned territories. Preferred Qualifications HomeCare HomeBase (HCHB) experience strongly preferred. IRS Mileage Reimbursement:

$0.76 per mile 2-week cancellation notice required. Any overtime must be pre-approved and accompanied by written approval from the Administrator.
