Position Summary The Licensed Vocational Nurse (LVN) provides direct nursing care to residents in accordance with applicable laws and regulations, physician orders, care plans, and facility policies and procedures. The LVN works collaboratively with the interdisciplinary team to promote quality care, resident safety, and positive outcomes. Essential Duties and Responsibilities Provide direct nursing care to residents, including medication administration, treatments, and monitoring of resident conditions. Observe, assess, and report changes in residents' physical or mental conditions to the appropriate healthcare provider or supervisor. Accurately document resident care, services, treatments, and observations in the medical record. Follow physician orders, individualized care plans, and established nursing standards. Supervise and provide direction to Certified Nursing Assistants (CNAs) and other assigned staff. Respond appropriately to emergencies and changes in resident conditions. Communicate effectively with residents, families, physicians, and members of the interdisciplinary team. Assist in maintaining compliance with federal, state, and facility regulations and policies. Promote a safe, respectful, and resident-centered environment. Qualifications Current and valid Texas LVN license in good standing. Current CPR certification, as required. Experience in long-term care or skilled nursing is preferred but not always required. Ability to communicate effectively and work collaboratively as part of a healthcare team. Ability to meet the physical requirements necessary to perform essential job duties.
