Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Manage complex cases involving acute pain management, dementia care, Alzheimer's care, and infectious disease protocols. Operate electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth for precise medical documentation and medical records management. Conduct diagnostic evaluations and coordinate discharge planning to ensure seamless transitions of care across settings. Provide specialized nursing interventions such as tube feeding management, ventilator support, and sterile processing techniques. Collaborate with multidisciplinary teams on case management, triage assessments, and emergency response procedures in high-pressure environments like Level I Trauma Centers or ICU units. Qualifications Valid licensure as an LPN or RN with current state licensing requirements. Proven experience in skilled nursing facilities or hospital settings with exposure to medical-surgical, ICU, emergency medicine, or long-term care environments. Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks; familiarity with ICD coding (ICD-9/10) is highly desirable. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to diverse patient populations including geriatrics and pediatrics. Experience working with individuals with disabilities or developmental delays; familiarity with dementia and memory care is a plus. Ability to perform advanced clinical procedures such as venipuncture, phlebotomy, suturing, airway management, and first aid. Excellent communication skills for caregiver interaction and patient education; commitment to maintaining clinical confidentiality policies in accordance with HIPAA regulations.
